Here are two recurring reasons why an email may not track an opening:

Some email recipients may prevent images from being downloaded or displayed. Since tracking works by adding an invisible image, it disables tracking. This action is not set by default, but it is possible in Gmail to require a "Requester confirmation before displaying external images".
Some browser extensions or additional software products may also block the tracking pixel’s loading.

Please also note that if you are sending yourself emails to test the tracking, this will not work. MailTracker prevents senders from triggering tracking events from their own emails.
